Coordinated family support (CFS) is a service designed for adults 18 and over with developmental disabilities who choose to live with their family.

CFS services will be tailored, individualized and flexible, incorporating preferences that best suit you and your family.

At TruHearts Support Services, our Coordinated Family Support Services (CFS) program is designed to provide comprehensive support to families caring for a loved one with intellectual or developmental disabilities. We understand that every family’s journey is unique, and our CFS program is here to ensure that families have the resources, guidance, and care coordination they need to foster a healthy supportive environment at home.

Our CFS program focuses on strengthening the family unit by providing tailored support services that address both the individual’s and the family's needs. We work closely with families to develop personalized service plans that empower them to care for their loved ones while promoting independence, community integration, and emotional well-being. We aim to relieve some of the pressures families face by ensuring they receive the assistance, education, community resources and emotional support they need to thrive as a unit. 

Our goal is to create a network of support that empowers families and enhances the quality of life for individuals with disabilities.

• Coordination of Healthcare services, assist with scheduling services, including medical and other appointments such as department of motor vehicles, SSI appointments, ect.
• Providing Info

             Providing additional information or resources on the      consumer's diagnosis and   

             identified support.

• Determining Additional Support as needed

              Identifying back up providers or supports and providing support as needed.                        

•  Family & caregiver education/training

             Provide assistance and training for the consumer and their family in navigating  

             comprehensive services that are tailored to meet their unique needs and maximize 

             their independence.
